Fee waiver

I am trying to fill out the Fee waiver form for my mom.

My sister and I rent a house. My mother and I live on our side of the house and I pay our rent.
I also give her a monthly allowance. She does not work and I claim her as a dependant

She does not make money of her own.
I am not sure how to fill out the form.

Does that fact that I work disqualify her?

I would appreciate any advice.
