I-94 mistake, how to correct?

There is an incorrect date on a I-94 visa. We realize we must go to a local immigration office to make the correction. Can you please tell me what is meant by local office. Is that the city where the employee is working or is it the point of arrival into the US or the point of departure? What documents does he need to bring to make the correction? Can we obtain any necessary forms online?
