Fingerprint chart

When I inquired with Mumbai consulate about green card re-entry permit biometric (fingerprinting) requisites I was told that one of them is that I should get a fingerprint chart and that this is to be obtained from USCIS in US if I want to submit my fingerprint to the consulate office in Mumbai for the re-entry permit. Will some one please educate me as to what exactly this fingerprint chart is? Is it a form ?
