Non Immigrant Form to be filled out

Hi

When a visitor boards the plane to US, he/she needs to fill out a non immigrant form which they normally distribute in the plane. Does anyone know whaere can I download/get a copy of that form ?

My mother is scheduled to visit me in about two weeks and her comfort level with English is very low. If possible I want to populate all the fields in the form for her and fax/e-mail it to her, so this can become a fairly simple task for her. If somebody has a blank copy, I would apprecaite if you could fax it to me
