    I hope that someone here can be of some help. I have a young friend who has immigrated here from Vietnam.

    When her 10 year Green Card was issued, her last name was misspelled. She and her husband found out through the local field office that they need to send an I-90 form with Reason "d" checked in question 2. The document instructs that she return her Green Card with the form. She also sent a copy of her passport information page. And a copy of her old green card both show the correct spelling of her name. She also submitted a written statement that the name was misspelled on the new Green Card.

    Everything was returned to her saying that she did not submit documentation to prove her identity and correct name spelling.

    On her behalf I phoned USCIS and asked what documents does she need to send to prove her identity. I was told; "I can not tell you." I was told that if the person I was talking to told me any suggested documents she could be fired. That the information was not part of documentation that she can share with callers.

    Can someone tell me what documentation she needs and in what form. I know a certified marriage license could be one. But I'm sure she would be reluctant to send in her actual passport, or her actual drivers license. She's required to have her state cosmetology license displayed at work, so she can't send that.

    Can we copy those items and have them notarized stating they are true and accurate copies? Does that count as certification?

    She has supplied all the required USCIS numbers. Attached to her Green Card and immigrate standing. All previous documents supplied to USCIS and form submitted resulted in the name being spelled correctly. But USCIS seems to refuse to verify their own records.
