I-90 form to replace permanent residence card

Hello, I have a question in regards to the Part #3 Processinfg Information:

They are asking for Date of Admision as an immigrant or ASdjustment of Status. Is this the date indicated on my green card as "Residence since"?

If I send in my I-90 appliocation, what are the required supporting documents I need to send along with it? Per instructions it appears that only copy of my green card is required. When do I submit the 2x2 photo? Do I bring it for the biometric appointment?
