I-130 Petition-Evidence Documentation

Hello,

I received a request for evidence with my Form I130 petition. The letter said that I should return the green sheet, the letter, the evidence documentation and a yellow sheet to the USCIS. I don't have
a green sheet, should i return the other documents without the green sheet ?

We have no idea what those different color sheets you are referring to.
