Green card renewal help

Hi,

I have a 10 year greencard which is expiring in oct of this year. we had done a counsular processing and had been to mumbai counsulate for the interview and our port of entry was detroit.

I have questions about part 3 on I 90 form


question 3

The city or residence where you applied for an immigrant visa or adjustment of status

Question 4
Counsulate where immigrant visa was issued or USCIS office where your status was adjusted

question 5

IF you entered us with immigrant visa also complete the following IF you entered us with immigrant visa you must answer this question. List your destination in us at the time of admission and port of entry where admitted to us in the space provided . IF you were granted adjustment of status at the uscis office in us then skip this question and proceed to qestion 6.
