Go to an IRS office or Acceptance Agent to get copies of child's passport authenticated, and send that copy with W-7 form and income tax return to the IRS ITIN office in Texas.

You can send the original passport, without secondary documents, with the W-7 and 2013 Federal income tax return, if you want.
I would recommend, however, that you get a copy of the passprot ID page authenticated by an IRS office or Acceptance Agent and send that authenticated copy rather than the original passport (with the W-7 and 2013 Federal income tax return, to the IRIS ITIN address in Texas.
What about my reply(ies) confuses you?

When submitting Federal income tax returns with the W7 (ITIN application), you must paper file (not EFile) by mail to:
Internal Revenue Service
ITIN Operations
P O Box 149342
Austin, TX 78714-9342
What is different about the IRS site details? The address I gave above is from the W7 intstructions from the IRS website.

I think the information provided by you is outdated.
I found following information on irs site
You can submit copies of original documents if you do any of
the following.
Have the copies certified by the issuing agency.
Have the officers at U.S. Embassies and Consulates overseas
provide certification and authentication services. Contact the
Consular Section, American Citizens Services of the U.S.
Embassy or Consulate in advance to determine the hours of
operation for these services.
Certified copies. A certified document is one that the original
issuing agency provides and certifies as an exact copy of the
original document and contains an official stamped seal from the
Agency.
you can find following information on below url
It is mentioned that original passport or certified copy by issuing agency not with acceptable agent or local IRS office. There are some IRS office who reviews the original passport and returns same day.
But my question is for 4 year kid do i also need to do that much documentation.
Thanks

Mannish,
I'm afraid we are looking a this differently:
1. You cannot request an ITIN for a minor child if he is outside the U.S. (unless in Canada or Mexico).
2. Original or true copies of government documents must be certified by the issuing agency. Passports, however, cannot be certified as original by a Consualte or Embassy, because those entities did not issue the passports.
If your 4-year-old child is in the U.S., you can get a copy of his passport authenticated by going to an IRS office or a commercial IRS Acceptance Agent. If you child is not in the U.S., he is not eligible for an ITIN.
