Difference between visa refusal and petition denial

My company applied L1B Individual for me 1.5 years back and it got denied after RFE.
Now the same company is applying for L1B Blanket for me.

I have couple of queries.

1. What should I answer this question in DS-160?
Have you ever been refused a U.S. Visa, or been refused admission to the United States, or withdrawn your application for admission at the port of entry?Explain
(I am thinking that mine is petition denied case, not an visa refusal)

2. What are the chances of L1B blanket approval after individual being rejected (1.5 year back)?

2. Without going into specific details of that denial, it is very difficult to make general comments.

The denial letter says that "The record is insufficient to establish that the position involves a special or advanced level of knowledge".

So, the past rejection will create any problem this time?
What should I answer if the VO asks about my past rejection?
