Anti-fraud fee for L1B extension

Hi,

As mentioned in the title, do I need to pay the $500 anti-fraud fee while applying for the L1B extension (based on blanket petition)?

Also, is there a restriction on the bank? If yes, is Axis/Citi bank accepted?
