L-1 visa and i-94 expires on may-06-2015

Hi,

i came to USA for the first time on L-1b - intra company transfer visa and it expires on may-06-2015 and also i-94

To apply for extension,

1) How many pay stubs are required

2) Before how many months we should apply for extension

3) how are the chances these days for extension
