Clear steps in order for scheduling Visa Interview and Fee Payment

Hello All,

I am getting confused with information scattered across multiple websites.
Can anyone please guide me how much fees I need to pay for Self and Spouse Visa.

My Details
1. L1-B Blanket Visa (Confusion is somewhere it mentions $190+$500, whereas someplace it says only Fraud prevention fee i.e. $500 to be paid)
2. L2 for Spouse (somewhere I saw that no fees need to be paid).
3. How do I pay the fees

For L1 primary applicant, $500 fraud detection fees and $190 application fees.
For EACH dependent(L2) of the primary applicant, only $190 visa fees.
In an ideal scenario, your employer will be paying the application fees directly to US Consulate and you may be provided a DD with the local currency equivalent of the $500 fraud detection fee.

Alternatively, if you want to pay out of your pocket (which is highly unlikely as employer should pay for L1 visa), you can do the same after completing your application on US Travel Docs site.
