spouse's name different

Hello all,
Looking for guidance regarding the following.
My husband holds L1 and is currently in US. I want to travel with my daughter.
1) In his passport, my married name is given. Ex: anaya velcha.
2) But my passport carries my name as jaya velcha
3) will a notarised affidavit or gazzette hold good enough for me to travel to US on L2.
Since he is in US we don't have an option to get his passport corrected. What are my options?

Also i would like to know the L2 interview can be given in mumbai??

Whats the name on the marriage certificate?

Also, Indian passport can be corrected/modified in US too.
