Happened to stay beyond I94 by mistake

When a person come back to US after the vacation he/she is stamped as ADMIT UNTIL 2017 in the passport at POE but the I94 expiry date is OCT 2015 and the same is not noticed until June 2016.
Please advice what should be/ can be done to this case.

Why the discrepancy between Passport stamp and online I-94? Staying beyond ones I-94 expiry can have serious consequences, and potentially being barred from entry in future.
