DS-157 question

Hi,

My wife was on L2(with EAD) and was working as an independent contractor.

Spouse(getting 1099 as a contractor)--->Company X------>Company Y------->End Client

Then she had to change status from L2 to H4. We got the approval notice for H4.

1)
Now when me(H1) & my wife(H4) go for stamping, in my wife's DS-157....can I mention her employment history?

2)
If she can mention her employment history...

a) should we mention that she was an independent contractor and not an employee of any company and not to mention anyone's name for the Supervisor details?
or
b) should we mention that she was working for Company X and give the supervisor's name from this company?
or
c) should we mention that she was working for End Client and give the supervisor's name at End Client site?

Please clarify. Your quick response will be greatly appreciated.
