need help with l2 ead application

hi all,

i am here on l2 visa and my hubby is on l1A. to apply for EAD i have couple of questions, if someone can help it would be really great. i am writing the answers which falls best according to my knowledge, pls correct me if i am wrong.

1) which category do i fall under? (a) (18)
2) status at last entry? l1a spouse, l2 visa holder
3) current immigration status? l2 visa holder
4)whats the mailing address for IOWA?
5)how do someone mail that?


guys also help if someone knows the fields to put in check as i am very new to US and its system.
