Filing L-1A after after 1 year of cool-off period

This is my first post to this forum and really looking forward to get help from your experienced advise. I will also try my level best to provide the correct information based on my experience.
Let me briefly describe my case first and then I will come to questions.
I was on US on H-1B visa and used all my 6 years back in 2016 and in the meantime my firm filed green card application in EB2 category but for some reason my green card application got denied. My firm lawyers did not give me the exact reason of denial because of their privacy issues but they appealed for my case in BALCA later and through which I got 7th year extension on H-1B visa till May'2018. However my employer gave me an option to stay outside US for 1 year as a manager and come back on L-1A visa which will eventually help to speed up my Green Card process as well through EB1 category. So I followed my lawyer's suggestion and came back to India (my home country) back in Oct'2016 itself without using my left over H-1B visa. So I just completed my 1 year as a manager in Oct'2017 and my employer started the processing for my L-1A visa.
Now I am filling my DS-160 for my wife and myself and there are questions where I really need some expert advise.
1. There is a question about my Previous Employer in DS-160. Will it create any problem if I mention the same employer through which my Green Card got denied earlier?
2. There is another question ("Has anyone ever filed an immigrant petition on your behalf with the United States Citizenship and Immigration Services?") in DS-160. Should I mention "yes" in this question because US Consulate may start asking about my Green Card application and its denial. However I do not have full insights on the reason of denial.
3. Also, how should I answer if US Consulate asks me about the reason of coming to India? What should be the right answer as per your experience?
4. Is there anything that I should be worrying about during my US Consulate interview?
5. Is there anything that I should be taking care while filling DS-160?
