EAD renewal question ?

I'm renewing my EAD, i'm confused with below question.

15. Have you ever before applied for employment authorization from uscis ?

Dates : ???

I have applied with Texas Service Center last May/2016. Got approved in June-2016. Could you please help me on what i dates i need to put.

This says Dates not Date !
