L-2 Visa: DS160 for my Wife and 4 year kid

Dear All,

Thank you for your continuous support. I recently got my L-1 B visa stamped.
Now I want to apply L-2 Visa for my wife and Kid.

My kid is turning 4 years in June.
Query1: Do I need to fill a separate DS160 form for my kid as well?
Query2: If yes, then would He need to visit for Biometric and Visa appointment?

Please guide me here with your experience.
