Queries related L2 visa

Hi,

I am currently working in US on a L1 Visa. I am planning to get married in Dec this year, and I had some queries related to the Visa procedure.

1) I am from Mumbai and my fiancee (wife) is from Hyderabad. From where can i apply Visa for my wife (Hyderabad(Chennai) or Mumbai or both)

2) If I am planning to apply for the Visa from Mumbai, we need to send some documentsto the Mumbai consulate and it should reach to the consulate five days before the interview.
Can you please let me know what all documents we need to send.
Also please confirm if I need to send the marriage certificate and wedding album also.
