URGENT: L2-EAD paper filing questions

Folks,
I know this has been asked many a times in this forum, but I still have a few doubts.

I am planning to apply for L2 EAD through Paper filing and I will be in US for 5 days before traveling back to Canada.

Questions I have

a) What is the exact procedure to file L2 EAD ( Fill form 765, fees, what else??)
b) Do I have to book an appointment for finger printing?
c) What other formalities are needed.
d) Do I have to come to US again to complete any other process?

You should be in US before filing EAD .

Form I-765 should be filled online , it will ask for your I-94 and all.
After filing, you will get a notice to your US address in one month for Finger print appointment. You dont need to book they will give the date and time.

after that...u just need to wait..wait..wait...coz generally EAD will take 3 months...but these days they are taking more than 3 months.
