L2 - apply for EAD - renew or fresh one ???

In 2005 I was on L2 visa and received EAD and A#.
The EAD expired in Feb-2007 and my L2 visa also expired.

Now I am back to US again on a NEW L2 visa and want to apply for EAD. I have 2 question w.r.t completing the I-465 form:

1. Should I select "Permission to accept employment" or "Renewal of my permission to accept employment"? Plz note that my previous EAD has expired in Feb'2007 and I am on NEW L2 visa.

2. For line # 10 in the form, do I need to mention my previous A# (Alien Registration Number)?
Please clarify asap.

1. Permission to accept employment. My guess it does not matter much. It is not renewal from how I see it.

Thanks for your reply. As you adviced I have e-filed I-765 as "Permission to accept employment". But another question is since I had an EAD card earlier, should I mail the photocopy of it in supporting documents ? Please let me know.
