I94 not returned while leaving US- Urgent

I Went to US on Dec 27, 2003 on F1 , but due to personal reasons i came back, after speaking with my Admissions Office to defer my admissions and came back on Jan 13,2004. While I came, I did not return the I94 departure form. Recently When i went for my L1 Visa, consular officer issued me the visa but told me that my departure is not updated in the INS data base and that i might be held on my port of entry.she also advised me to ask UAH to inform INS that I did not register for the course.I joined my new office as soon as i came back to India on Jan 19,2004 . As I am likely to travel within two weeks, please let me know what i should do and how to contact INS regarding the same.
