L1B Visa queries

I am appearing for L1B Blanket visa next month in chennai along with my dependent Spouse.
I wanted to know the following:

a) I have Biometrics appointment a day before. Do we need to carry anything other that the Passport/DS160 for this appointment?
b) For the Visa interview appointment, what is the right attire - Just formal Shirt/Trousers would do or is Tie/Blazers mandatory?

Technically you will just need the passport & DS160 confirmation page, but, it would not harm if you carry all your documents (in case you need them).
Formals should be good enough. Tie/Blazer is also ok.

Hi I have my L1B Visa scheduled for early next month. I have a few quick questions as below :

We have to pay Fraud Prevention Fee , what is the most accepted mode of payment for this fee? Can we use Credit Card or a Demand Draft is necessary? I heard there can be issues with Card Machines at the center.

Also, what all documents do I need to carry for this interview? is there any specific categorization of docs I need to prepare before I enter the premise?

I have my spouse accompanying me for this interview, what all documents do I need to carry for him? He was once denied a B1/B2 visa back in 2011, do you think that plays a major role? if asked about the same what will be the right answer?

Have a follow up query, for the DD for Anti Fraud fee, it has to be in the favour of US Consulate General or American Consulate General? Most forums talk about the US while USTraveldocs website says American. Also, the DD is supposed to be in INR or in USD?
