L2 Visa questions

Hi,
I am in USA on L1B visa. I am going to get married soon. I have few questions while i request for my spouse L2 visa:

1. Can we submit DS-160 form even before actual marriage date? So that we can block a PA date in advance.
2. Do we need to add spouse name in any of our passports?
3. Do we need to change surname in my spouse's passport?
4. We are from AP. Can we book PA date in Chennai as currently there are no dates available in Hyderabad?

Please help me in clarifying these questions.

>>> Yes. You need to be married by the time you attend the interview.

2. Do we need to add spouse name in any of our passports?
>>> Not required. Marriage certificate, marriage invitation and few marriage photographs are enough to prove the relationship.

3. Do we need to change surname in my spouse's passport?
>>> No. Read the above answer.

4. We are from AP. Can we book PA date in Chennai as currently there are no dates available in Hyderabad?
>>> Check with VFS chennai.
