Query for L2 Visa & Marriage Certificate-Name Different

Hi,

My name is written different on my marrieage certificate than on my passport.

I am currently in US and want to aply L2 visa for my husband.

Please tell me how to approach.

1. Is bride's maiden name suppose to be present on marriage certificate?
2. Is there a way to get name changed on marriage certifiacte?
3. Shall I be required personally to get the name changed?
4. Shall I get an affidavit stating that name on passport & name on
marraige certificate belong to same person?
