221 g for passport damage case

Hi,

I got 221 (g) for passport damage case. on the 221 g its requested that - please obtain a new passport.

Question 1:

Do i need to submit I797 copy as well with my old and new passport?
Or it's ok to submit old and new passport only.

Question 2:

Also on 221 (g) form that i have downloaded from


Passport Number: ___________________________________


Do i need to give both the passport numbers OLD and NEW one with comma seperation in above box?

Got the answer from VFS office itself...submitted my old and new passport along with original 221 (G) + I 797 copy
