Renewing my B1 what to put on Date Last Visa Was Issued if I had a J1 after my B1?

Hi, I am applying to renew my tourist visa to US (B1/B2), but I had another visa (J1) issued during my first B1 visa validity.

What should I put on the question Date Last Visa Was Issued? Info about the B1 visa or the last one (J1)?

I am asking it because it does not have the option to include more visas, and the next question is about if the type of visa that I am applying now is the same that the one stated above.
