Hi my boy friend is considering applying for F-1 visa he currently lives in Italy. We are wondering in the interview if they ask if he is in a relationship whether to answer honestly or say that he is not? To our understanding if he says that he has a girl friend that they will deny him the F-1 visa, because the intent to get married?

That's kind of an odd question to ask during a F-1 visa interview. Unless he has visited you (=the area were the university is located) many times or for prolonged durations, so that they might assume he has more ties to the US than Italy. I would never lie in such an interview, but if asked about US girlfriend he could emphasize that your relationship is not that evolved yet that you are considering marriage.
