DS160 Form H4-F1 questions.

Hello all,

I am planning to go to India for my F1 Visa Stamping( I am on H4 visa). I already enrolled in school from spring 2013 and pursuing master's on H4 but I recently received Graduate Assistant-ship starting from Fall 2013, so planning to convert to F1 by going to India and get F1 Visa stamping. I am having couple of questions in filling DS 160 form, please advice.

1. I am already staying in USA for the last year, So, Home address/Home Phone should I put in DS160 form.
2. I am already pursuing my master's on H4 visa and completed two semesters(Spring,Summer),So, in the educational details, Do I need to enter these details.
I am not able enter these details because I cannot enter 'Program End Date ' as future date(I did not complete my master's yet). So, my question is do I need to submit these details or not?

Please advice, if anybody has done this before. Thank you.
