H4 to F1 COS: In I-539 form, what is answer of requested status be extended until.

Hello All,

I am currently on H4 visa, which ends on 27th August, 2016.
I want to apply for Change of Status from H4 to F1.

While filling the I-539 form, I came across below question, I am really not sure what should be the answer.
Question:
Current or requested status be extended until extended until (mm/dd/yyyy)
1. Should I write my Visa or i-94 date end date
2. Should I calculate 24 months(2 yr course) from the start date
3. Should calculate date like below:
My course is under STEM, so total duration of months can be
24 Months (2 yr course)
+ 12 Months(OPT)
+ 17 Months (OPT extension)
= 53 Months , 4.5 years almost
Please guide me, I am really confused.

You should talk to your international student adviser. They will be able to guide you best. My adviser asked me to leave it blank but it was for a phd program of 5 years duration...so talk to your advisor...good luck
