Traveling to India on OPT and EAD without a job

Hi,
I need to travel to India on medical emergency. I am on OPT and have an EAD since January 2016 and. I have joined a consultancy and still in marketing.
How can I come back if I go to India now. Will my consultancy employment letter be sufficient for re-entry in OPT. Will my OPT term end ? Will it depend on the university I did my Masters.
And should I also be giving them a notice of my travel ?
Please advise. Any help is appreciated.
