W9 on OPT STEM EAD

Hi All

I was an international student. Currently, I have applied for H4 COS as of end of August 2018.

I was working under an employer for period of 8 days during first week of June and I was paid for that period under status F1 OPT. The employer has now sent me a W9 to fill up and send them back to me. As I remember, I filled W9 long back to get my SSN. Can someone share their experience ?
