First and last name swapped in Visa. Need to apply for H1B this year.

After losing my passport in the USA, I applied for a new one through Indian consulate in Houston and got a visa done in India. It took me a while to realize that the first and the last name were swapped. So, I got a new passport made and got I-94 fixed. I am looking forward to apply for H1b visa this April. How should I get a visa matter fixed?
