H 1 Visa requirements for Medical doctors

Wanted to know
1. Does a medical student have to have passes the USMLE step 3 to be eligible for the H 1 Visa

2. If, I enter the US on J 1 Visa for 1 yr, can i continue on the J 1 visa, if I have got admission elsewhere?
