Please help

Hi , I am at present on a J1 research visa in US.
My wife is pregnant at present and my Visa will get over at the end of July.
That time I will be getting a clinical J1 visa, sposnsored through ECFMG.
and my wife will be six months pregnant.
So will she have a problem in getting a visa?
Can the conversion be done in US itself?
Is there any other alternative?
please let me know if someone has any experience on this.
