query on F-2 visa

i would be applying for F-2 visa shortly
could anyone help me with some of my queries-----
1) which r the documents needed for the interview for F-2 visa
2) if once it gets rejected how soon can one apply for it again and for maximum how many times
3) how much bank account does my husband have to show as a support
4) on which common grounds does F-2 visa application gets rejected

There was a memo to consular officers some time ago that if a F1 visa holder qualified for a non-mmigrant intent, his dependent should not be rejected on the basis of immigrant intent.
