Confusion with Surname in DS-156 form !!!!

Hi

I am applying for a Student F1 visa ..

My Passport has the following information

Surname :
Given Name : Anand Krishna Shankar

Pls tell me what should I fill in the boxes 6 and 7 in the DS-156 form

BOX 6 Surname(As in Passport) :
BOX 7 First and Middle Names (As in Passport) :

Your suggestions will be invaluable .Pls respond if you have encountered a similar situation before..

VFS instructions say put FNU in First Name
Put entire Given name as Last name
