    hello guys,
    i am working on OPT right now. I know that i do not have to pay medicare and social security taxes, but my employer needs a form or application that states this matter.
    i gave him a copy of irs publication "This exemption from social security and Medicare taxes also applies to employment performed under Curricular Practical Training and Optional Practical Training, on or off campus, by foreign students in "F-1," "J-1," "M-1" or "Q" nonimmigrant status as long as the employment is authorized by the U.S. Citizenship and Immigration Service." and my OPT card.


    But, he is not satisfied... He needs a form or application, but he doesnt know what kind...he doesn't know what he's talking about.
    Any suggestions? Form?

    They will need proof of your current F1 status such as visa, I-94 and I-20 for their records.
