COS: H4 to F1 (submit original or copy of I-94?)

Hi All,

My wife is currently on H4 visa, and has gotten admission at a local university. We are filing a change of status application for her (H4 to F1). I am not sure if we need to submit the original I-94 card with the application, or just a copy (front and back) of the I-94 card.

I have gotten conflicting versions on the web. I would really appreciate if somebody could give a definitive answer based on past experience.

I applied for my wife's F1 COS and submitted original I-94 since you get a new I-94 with F1 approval

Send only copy of I-94. Never send original.
