Documents required for H4 to F1 COS

Hello all,

I need to change my status from H4 to F1 to study in a business school in US. I got the admission, I-20, paid the fee for I-901 and e-filed I-539. Now I need to know what supporting documents should I send in to USCIS to process my COS?
By going around tons of google searches, I found below list of docs:

1. I-539 e-file confirmation
2. I-901 receipt (I paid online, so have confirmation page)
3. Original I-20
4. Original I-94
5. Bank statements to show sufficient funds for the study per my I-20
6. Letter of consent, my intention of study and return to my home country
7. Letter of sponsorship from my husband
8. Copy of my passport (visa and identity pages)
9. Copy of my husband's H1B I-797, passport

Is there anything else that I need to submit along with those listed?

Also, anybody has sample letters that helps me?

Add spouse's last 2 or 3 month's paystubs and copy of spouse's passport and Visa (if any).
Your own cover letter should mention that you have maintained residence in your home country, if possible with address.
Keep copy of I-94 (both sides) with you for your records, after you submit the original. In fact keep copies of everything that you submit.

Can i use I134 for COS from H4 to F1

Hi

Can we use I134 form as affidavit of support fro financial guaranty statement for COS process from H4 to F1..I am asking this because in few forums it was mentioned that this form is no longer accepted.

Also I would like to know If it is enough to show my sponsor's last three months bank statements in support with this form?

Just bank statements are sufficient. I-134 is not required.

I have prepared the list based on your suggestions. Please let me know the address to which these documents need to be mailed and also please let me know if we need to send the original I-94 and original I-20.

I20 can be submitted in original. School DSO can easily generate another if required. I-94 need not be submitted in original IMO.

My DSO asked me to fill I-134 Affidavit of support as well.
Also, you need to present the proof of everything mentioned in I-134 like your husband's bank statements, stocks, other financial assets.
Apart from that my husband also submitted letter of sponsorship for me.
