Visa for Grandmother over 80

Hi All,
My grandmother is 80 plus and I would like to apply visa for her.She doent have to attend visa interview and she can submit documents at VFS office. When all her documents have been submitted they asked her to submit checklist which we are not aware of. Can you please guide me as where I can get the checklist document.
