Help with DS157 form filling

Hi all,
What to write in Point 12 of form DS-157, as I am still with my first job, and the field says, " Not including current employer, list your last two employers"

should I leave it blank?
or
Should I mention my last occupation as "owner" and other details, when I was self employed

If you are self employed, your employer is 'yourself'.

Therefore, write accordingly.

actually I dont know exactly how to get in touch with you directly.
its has been said that age between 16 and 45 must fill the form DS157, I am over 45, my dob is 3 march 1960,

Until you are 46, you are 45.

That means, you need to fill that form.

good morning, thank you very much for your reply, I really apreciate your cooperation.can I have your direct contact so I can talk to you freely. be honest with you, I am in such situation that I dont think I can get visa, If I was consulate I would not give visa to my self.if I submit the papper as I did last year, I was rejected
