Can my nephew travel outside of US while on B2 multiple entry 10 yr visitor visa?

My nephew is here visiting us for a 5th time. He has a 10 yr multiple entry visa. If we wanted to take him with us on vacation to Canada or Carribean, can he come? Would he be allowed back into the US without a problem? I realize that he may have to apply for a entry visa to the country we'd be visiting.

Yes, no problems.
But he will have to get a canada/carribean visa first.
