Visitors visa for grandparents and brother for assistance

Hello Guys,

I am sponsoring my grandparents for a visitor visa (they are in 70's) and also planning to sponsor my younger brother (26 years old) for assisting them during their travel. The questions that i have is...

1. Do i need to have separate I-134 form for my Grandparent and Brother? (I was planning to have one form for all, saying my brother as grand child instead of child)
2. If its separate form, How would the visa officer would know that my brother is applying the visa so that he can assist my grandparents.
3. Is it valid to say that the sponsoring of Visitor visa for my brother is solely due to assisting my grandparents?
