Name swapped in passport

My parents are traveling from India to USA in month of June. My father got his passport renewed in India last year. Then the passport office has swapped his first and last name without any request from him. His USA visa is in his old passport which they have attached to the new passport. My father will be presenting both the passports during his travel. With this background I have the following questions:

1. As the USA visa is in his old passport, will there be any issue at immigration services at indian airport and port of entry in USA?
2. Because of the name swap between his old passport(also his USA visa) and new passport, will he be facing any issues with immigration services at indian airport and port of entry in USA? Can you suggest if he can carry any type of documents like a notarized letter which might help him during travel?
3. What name should we use while booking his ticket?

I need a quick reply as I need to book their tickets today.
