I-134 form filling doubts

My husband is working in an IT wit designation as project associate. While filling 134 form,
1. For the question "7. I am employed as or engaged in the business of -------" what we shud mention? Whether as "software engineer" or designation shud be mentioned?
2. for the same question but for the location "at -------city, Il" whtr we shud mention the working location address or the company head office address?

Please guide.

Thanks for the reply. And I have a query reg bank letter. Whtr letter from the bank can be one month before the visa interview date? ex if visa int is scheduled on jun 15, whtr bank letter can hav d dated on may 10?

no hard and fast rule.
you can also send the most recent bank statement.
they probably won't look at all that, they will just glance over the I-134
figures and decide.
