Getting Visa in Chennaii

My mom has travelled to USA 2 times and both the times she had 6 months Visa only, however once we extended her stay by applying for the extension and she got the extension and she left before the due date as well. However we want her to visit us again, she needs to go to Chennaii to get the Visa, my question is does she need a Extension letter which she had earlier, if so we do not have that one now, how do we get that?

Also I heard that one family though their extension was legal, when they came next time the Computer did not show anything about their extension and they were not permitted to enter US and were held back in LosAngeles airport, could some one advice what we should do? Thanks

1. Yes, extension notice is needed.

2. As long as you are carrying the copy of extension, that is the proof that it was legal.
