2nd Visit after overstay

My wife is going to the US for the 2nd time in 7 months. Last time she had overstayed for 5 weeks, but had applied for permission which was granted on medical grounds. What are her chances this time around. Any advises?

Your situation is not clear. What visa she had before and what visa
does she have now? What do you mean by permission ? Did she apply for
an extension on med. grounds or did she apply for another visa on medical grounds?

Sorry for not being clear. Actually she had been there on a B2 visa and had to overstay due to pregnancy complications and our child was born there. She had a return ticket which lapsed unused as her doctor there advised her not to travel. She had applied for an extension which was granted, but she overstayed only for 6 weeks and then returned. This time she will again apply for a B2 Visa. Her parents are Permanent Legal Residents there.

If she returned before the date on the extension,
she is fine. She will need to take the extension approval notice to the interview , and the doctor's papers from her last visit.
